How to Choose a Public Health Dissertation Topic in the UK (2026)

Short answer: One fact shapes every UK public health dissertation. The Health Research Authority does not accept applications from undergraduate students at all, and accepts master’s applications only from students on health or care programmes in research-active departments. Even then, the supervisor is normally the Chief Investigator, not the student. That is why the systematic or scoping review is the default design in this discipline, and why a review done properly is the strongest route available to you rather than a fallback.

The HRA wall, stated plainly

The HRA’s published position is that health and social care research applications from undergraduate students are no longer accepted for Research Ethics Committee review, for HRA and HCRW Approval, or for study-wide review in Scotland and Northern Ireland. Master’s eligibility is restricted and must be established through the HRA Student Research Toolkit before any application. Students on non-health degrees are not eligible. Doctoral students remain eligible and can act as Chief Investigator. These criteria took effect in September 2020.

The misconception that costs students most is thinking their project falls outside this because it is “just a survey of nurses” or “just a few interviews with patients in a waiting room”. Recruiting NHS staff as research participants about their work engages NHS research governance, and so does recruiting patients. Neither is a way round the wall.

The salvage route, and why to decide early

Universities sometimes rescue a stalled project by reclassifying it as service evaluation or audit, which does not need REC review. This is legitimate, but it changes the research question. Service evaluation asks whether a service is meeting its own standards; research asks a generalisable question. A project designed as research and relabelled as audit in month four usually fails on methodological coherence, because the aims, the sampling logic and the discussion no longer fit together.

Decide at proposal stage which of the three you are doing: a review, a secondary analysis of open data, or a service evaluation. All three are respectable. Switching between them late is what causes the damage.

Doing the review properly

A review is not a long essay about a topic. What distinguishes a marked-well review from a marked-poorly one is entirely procedural.

  • Register the protocol. PROSPERO for systematic reviews of health-related outcomes, or OSF where PROSPERO is not appropriate. Registration before screening is what makes the review reproducible rather than retrospective.
  • Report against PRISMA 2020, or PRISMA-ScR for a scoping review. Reproduce the full search strategy for at least one database, with the date run and the number of hits.
  • Appraise formally. CASP checklists are the UK norm at this level; JBI tools and AMSTAR 2 where they fit better.
  • Synthesise, do not list. A results section that walks through studies one at a time is the most common structural failure. Organise by finding, not by paper.

Before you commit, run the search. Fewer than roughly eight relevant papers and there is nothing to review; several hundred and the question is too broad. Ten minutes in Medline and CINAHL at proposal stage prevents most topic changes.

Fingertips is the most underused of these. Deprivation-stratified indicators at local authority level mean a genuine health inequalities analysis with no access negotiation at all.

One practical warning: NHS England is being merged into the Department of Health and Social Care, with completion targeted around October 2026. Statistical publication URLs and responsible body names are likely to move mid-project. Archive the page you cite, record the date you downloaded, and keep a local copy of every dataset.

Live territory for 2026-27

  • Tobacco and Vapes Act 2026, Royal Assent 29 April 2026. Bans sale of tobacco to anyone born on or after 1 January 2009, bans vape advertising and sponsorship, restricts vape packaging and display, and creates retail licensing powers. Implementation dates for individual measures are still being set, so the evaluation literature is thin and the policy debate is live.
  • The 10 Year Health Plan for England, “Fit for the Future” (July 2025) and its three shifts: hospital to community, analogue to digital, sickness to prevention. It effectively supersedes the Major Conditions Strategy, which never appeared as a standalone document.
  • The National Cancer Plan for England, published February 2026, with clinicians publicly questioning whether its timetable is deliverable. Contested, evidenced, and recent.
  • Health inequalities remains the highest-yield area precisely because the data is free, granular and already stratified.
